An Application Form

Whats up guys. I’ve been quiet for some time now, out of web dev for just as long. Luckily you can count on me to come back and torment the lot of you!

Basically, I am creating an application form for a client.
Usual stuff, a bunch of input boxes. Textareas, some radio options and a checkbox. It’s for an event booking thing.

Thing is, I have no idea what I’m doing past this. I’ve made the form template and that’s all tidy, all named/id’d correctly.
Normally, I’m used to contact style forms… That’s how I’d operate this but the client wants a kind of login area where they can view the applications, etc.

So.. Essentially I need to link this form to a DB and provide output pages for every separate application. What would be the best way to go about this?
I’ve a good understanding of PHP, having worked with forums in the past, I just need to grasp the whole output and archive thing really.
